The following York County restaurants were found to have violated Pennsylvania's health and safety regulations during inspections between March 3-9.

Inspections are overseen by the Department of Agriculture. The department notes that in many cases violations are corrected by the restaurant before inspectors leave.

--Prime rib roast not reheated properly The roast was placed in the oven on site.

--No clean-up procedure for diarrhea and vomiting events.

--Container of chili in refrigerator as date marked and date-of-use had expired.

--All refrigerator units are required to have thermometer.

--Door seals on basement walk-in cooler in disrepair, flooring in walk-in cooler is rough and not easily cleanable.

--Dirt build-up on food chopper.

--Time in lieu of temperature being used as a control for potentially hazardous foods, but rice for sushi observed being held for more than four hours.

--Observed heavy accumulation of frozen condensate on interior surfaces of chest freezer located directly beside rear exit door.

--Torn/loose rubber door gaskets observed on the walk-in cooling unit.

--The wash-solution temperature in the mechanical ware-wash equipment sanitizing with chemicals was 86 F, rather than not less then 120 F, as required.

--Observed broken/disconnected wires on fryer basket.

--Several raw animal foods such as chicken, beef, shrimp were stored above ready-to-eat sauces and produce in the walk-in cooler.

--Observed several cases of meat such as chicken, tuna and shrimp stored directly on the floor inside walk-in freezer, rather than six inches off of the floor as required.

--Observed two boxes of avocados stored directly on the floor in dry storage area, rather than six inches off of the floor as required.

--Walk-in-cooler floor, a non-food-contact surface exposed to splash, spillage, or other food soiling, is rusted, corroded and pitted and is no longer non-absorbent, smooth and cleanable as required.

--Wood legs shelving under three-basin sink supporting chemicals are not smooth, non-absorbent, corrosion resistant.

--Mechanical ware-washing equipment observed with build up of filth and food residue and not cleaned before use, and frequently throughout the day.

--Non-food-contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed excessive oil, food and debris between and on sides of fryer and wok cooker.

--Observed wet wiping cloths in cooks line area, not being stored in sanitizer solution.

KIMMY'S CAFE, LLC

--Observed wall/ceiling over-top of three-compartment sink is chipping/split and is not an easily cleanable material

--Observed walk-in cooler flooring having food and trash debris and in need of cleaning.

--Observed numerous employees with full facial hair or goatees involved in food preparation not wearing beard covers as required.

--Observed duct or electrical tape wrapped around a door handle on the upper half of the refrigerated bain-marie.

--Ventilation exhaust hood above the grills and deep fat fryers is missing hood filters.

--Time in lieu of temperature being used in the food facility to control ready-to-eat potentially hazardous foods without written procedures or documentation to verify disposition of food. Observed cooked chicken tenders being stored and held at room temperature during lunch service without documenting disposition time when not using heat or refrigeration as a food-safety-control measure. Corrected on the spot.

--The quaternary ammonia concentration in the sanitizing solution of the three-bay ware-wash sink was 50 ppm, rather than 200 ppm as stated on the manufacturers use directions. Corrected on the spot.

--Equipment, utensils, durability. Observed dented, bent and cracked or broken in various locations on a metal colander full of cut, washed lettuce.

--Materials, non-food contact, durability. Observed broken laminate counter-top at the hand-wash sink behind the customer service counter.

--Equipment, utensils, durability. Observed broken, chipped and cracked plastic food-prep/storage containers on shelving with other clean equipment in the ware-washing area.

--Surface characteristics of Indoor areas of the food facility. Observed heavily stained raw concrete floors in the short order cooking area and in the ware-washing room. Raw concrete is absorbent and not cleanable.

--Observed beef jerky, drinks, product reclamation stored directly on the floor in storage area, rather than six inches off of the floor as required.

--Observed external underside of milkshake machine with an accumulation food residue on non-food-contact surface.

--Fan covers inside walk-in cooler are not cleaned at a frequency to preclude accumulation of static dust.

--The quaternary ammonia concentration in the sanitizing solution of the wiping-cloth bucket located in the restaurant area was 50 ppm, rather than 200 as stated on the manufacturers use directions.

--The water temperature of the wash compartment of the manual ware-washing equipment was 86 F, rather than not less than 110 F, as required.

--Floor drain under the three-basin sink is extremely dirty and in need of cleaning.

Inspection type: Regular (Out of compliance)

--Person-in-charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions related to proper ware-washing, possible physical/chemical contamination of foods and proper cleaning of hand and ware-washing sinks.

--Person-in-charge does not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

--Observed case of single-service drink cup lids directly on the floor, and not six inches above the floor.

--Food employees observed involved in drink and food preparation not wearing proper hair restraints, such as nets and/or hats.

--Squirt bottle of water used to spray espresso machine whipping wand is not labeled.

--The food facility does not have the original certificate for the certified food employee posted in public view.

--Broom and long-handled dust pan stored between doughnut case and shelving with coffee brewer and single-use items with potential to contaminate food equipment and single-use items.

--The quaternary ammonia concentration in the sanitizing solution of the three-bay ware-wash sink was below 50 ppm, rather than 150-400 ppm as stated on the manufacturers use directions. Discussed manually mixing until repaired.

--Manual ware-washing equipment observed with build up of filth and oily residue and not cleaned before use, and frequently throughout the day.

--Interior floor of walk-in freezer was observed to have fallen foods, food crumbs, plastic, tape etc.

--Food employee observed rubbing face with back of gloved hand potentially contaminating and continued to prepare sandwich. Discussed with manager.

--Food employee observed donning single-use gloves without a prior hand-wash.

--Observed plastic/silicone baking pans being used in daily food-prep activities, which are torn and frayed and not durable under this usage condition.

--Brown oily residue residue observed in the hand-wash sink, indicating uses other than hand-washing.

--A working container of Dawn all purpose cleaner was stored on food-preparation table with food, equipment and utensils.

--Coffee machine cleaning tablets stored on counter top/drink preparation table.

EDIBLE ARRANGEMENTS - VILLAGE GREEN

2300 E. Market St., Suite 9B, York

--Observed insecticide Ortho home defense in the back storage room area, not labeled by the manufacturer as approved for use in a food facility.

--Mops are not being hung to air dry.

--Time in lieu of temperature being used in the food facility to control ready-to-eat potentially hazardous fresh brewed tea without written procedures or documentation to verify disposition of food.

--Food containers stacked and stored as clean, a food-contact surfaces, was observed to have stickers/sticker residue and were not clean to sight and touch.

--Wall behind oven near dish area, has a hole and is in need of repair.

--Light shield directly over the food slicer is cracked. Other light fixtures in the food-preparation area are not completely shielded to prevent contamination in the event they break. Manager did not know if the bulbs are shatter resistant. She will ask maintenance department and take corrective action based on bulb type.

MAMMA'S PIZZA & SUBS & DINNER INC

--Lights are not shielded or shatterproof over ware-washing and food-prep areas.

--Observed bottom shelving of pizza-prep refrigerator unit with a large accumulation of old-food and soil residue and in need of cleaning at this time.

--Observed refrigerator units located on cooks line, non-food-contact surfaces not cleaned at a frequency to preclude large accumulation of dirt and soil.

--Plumbing system not maintained in good repair - observed three-compartment sink leaking at the bottom PVC piping area and in need of repair.

--Can opener/microwave/utensil (clean container holder), a food-contact surface, was observed to have old caked on and black encrusted food residue and was not clean to sight and touch at the time of inspection.

--Temperature-measuring device for ensuring proper temperature of equipment is not available or readily accessible in all refrigeration units at the time of inspection.

--Observed single-service, single-use articles (pizza boxes) stored in dry storage area directly on the floor, and not six inches above the floor.

--Observed a chemical bottle with no identifying label.

--Drink lid holders in the customer service station are stained and have an accumulation of residue.

--Floor in the dishwasher area is heavily stained.

--White plastic bins with drawers that hold the cooking utensils in the kitchen are stained and need cleaned.

--The hand-wash sink near the dishwasher is bent forward and not sloped to allow for self-draining. Water is pooling in the front of the sink.

SOUTH CENTRAL YORK COUNTY SENIOR CENTER

150 E. Main St., New Freedom

--Observed flooring in rear dining room area (below windows) has holes from the removal of equipment and needs to be repaired at this time.

--Food storage bins were stored stacked and wet on the shelf.

--Microwave oven had food debris and spatter in sides and top. This item was corrected on-site. Spatulas had food soil but were corrected on site.

--Mop sink spigot had an attached hose with discharge end lying on the bottom of the sink negating an air gap to prevent back flow.

--Observed floor area and PVC piping underneath three-compartment sink and around grease trap has an accumulation of soil, dirt, food and trash debris and in need of cleaning at this time.

--Food facility person-in-charge was not able to provide documentation that prepackaged duck (raw) are from an approved source.

--Seed and bean bins need scoops with handles to prevent contamination of product from hand contact on dispensing cups.

--Prepackaged duck (raw) is not labeled properly with the name of product, net weight and safe handling instructions as required for raw animal products.

--Prepackaged fruits, nuts, seaweed and mushrooms is not labeled with the name of product and net weight.

--All-gender toilet room is not provided with a covered waste receptacle for sanitary napkins.

--Exit door located in the rear area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Manager stated the door sweep has been received and will be attached within two days.

--Observed shrimp thawing in standing water in the three-compartment sink, which is not an approved thawing method.

--Ceiling tiles above three-compartment sink are heavily soiled with large brown spots and need replaced.

--Observed food in upright refrigerator and freezer units are stored open with no covering.

--Food utensils located on prep table was observed stored inside a container of water which is not maintained at 135 F. Temp of water during inspection was 65 F.

--Observed chicken (food) stored directly on the floor in walk-in cooler area, rather than six inches off of the floor as required.

--Plumbing system not maintained in good repair - observed three-compartment sink leaking at the bottom PVC piping and in need of repair.

--Observed equipment hoses, pipes, attachments, in cooks line area, with an accumulation of dust, dirt, food residue, debris on non-food-contact surfaces.

--Observed pooling water from previous night floor cleaning over-spray on shelf below food-preparation table.

--Observed oven interior with encrusted grease and food-debris accumulation.

--Plumbing system not maintained in good repair - observed three-basin sink, two hand-sinks and hose to floor cleaner leaking at the faucet, spigot and hose connections.

--Observed frozen condensate covering approximately 1/8 of bottom inside under-counter freezer located beside ice cream freezer on cooks line.

--Ansul system pipes directly over flat grill a food-contact surface, was observed to have food and grease residue and was not clean to sight and touch.

--The hand-wash sink in the front preparation area was blocked by stacked dish racks and not accessible at all times for employee use.

--Ceiling intake-vent cover directly outside of dish area is extremely dusty.

--Observed in-use spray bottle of vegetable oil hanging on Ansul system piping creating possibility for contamination of hands when used.

234 N. Front St., Bldg, 3, Wrightsville

--Observed a spongy slimy mold-like growth in the small crevices of a removable plate attached to a control arm that holds a thermometer which regulates ice production via temperature inside the bulk ice machine. Corrected on the spot.

--Observed cases of popcorn and Bazooka bubble gum stored directly on the floor in dry storage and projector-room storage area, rather than six inches off of the floor as required.

--Observed cases of drink cups - single-service articles stored in dry-storage area directly on the floor, and not six inches above the floor.

--Observed numerous food items within walk-in freezer are stored open with no covering.

--Observed in-use knives and/or cleavers stored between table edges or between tables, an area not easily cleanable and sanitized.

--Observed boxes of food stored directly on the floor in walk-in freezer area, rather than six inches off of the floor as required.

--Observed the underside of the kitchen prep table, non-food-contact surfaces not cleaned at a frequency to preclude a large accumulation of dirt and soil.

--Observed hood vents over-top of stove/grill area has a large accumulation encrusted grease and black soot and in need of immediate cleaning.

--Observed floor drains and the entire flooring located - in the rear kitchen a area of the food facility is extremely dirty, dusty, and in need of cleaning.

--Observed flooring within walk-in freezer area of the food facility is extremely dirty, dusty, and in need of cleaning.

--Observed clean dish racks stored on the floor in the ware-washing area at the time of inspection.

--Observed one of two side concession stand entrance-exit doors that does not form a light tight seal when in the closed position.

--Food facility is using an approved non-public water system, but does not have current laboratory testing results for water potability. Water analysis can be emailed to mneff@pa.gov or faxed to 717-346-3229.

--Observed packaged store-made vegetable trays packaged with ranch dressing displayed for sale customer self-service from the reach-in refrigerator that lack ingredient labels for the ranch dressing. Labels must contain a list of ingredients (including sub ingredients) in descending order of predominance of weight. This includes a declaration of artificial colors, flavors and chemical preservative if contained in the food. Accurate declaration of net quantity, name and place of business of the manufacturer and major allergens in applicable.

--Observed packaged store-made bologna-and-cheese sandwiches and creamy vegetable soup displayed for sale, customer self-service from the reach-in refrigerator that lack ingredient labels. Labels must contain a list of ingredients (including sub ingredients) in descending order of predominance of weight. --Observed 12-15 old rodent-like (mouse) droppings inside the cabinet underneath the carbonated beverage dispensing unit on the retail floor.

--Fry-dump heat lamp and casing directly over the ready-to-eat fries, a food-contact surface, was observed to have food residue and was not clean to sight and touch. Observed grey/white/pink substance on customer self-serve fountain soda machine dispensing nozzles.

--Observed oil filtering units in the storage area, with encrusted grease and soil accumulation.

--Observed equipment casters, in the kitchen area, with encrusted grease and soil accumulation.

--Observed heavy accumulation of static dust on interior wall of walk-in cooler.

--Floor under the fryers extremely dirty, dusty, and in need of cleaning.

--Food employee observed changing tasks that may have contaminated gloved hands without a proper hand-wash and glove-change in-between.

--Food employee observed storing clean food equipment while wet, and not allowing time for draining and/or air-drying.

--Floor of the mop sink area is made of tile and is cracked/roughened/and is not a smooth, easily cleanable surface.

--Severely dented canned chicken breast observed in sales area and intended for sale in the food facility.

--Observed discontinued chemicals and pesticides being stored in shopping cart in direct contact with candies, drinks and various other foods.

--Observed canned/bottled drinks stored directly on the floor in storage and sales areas, rather than six inches off of the floor as required.

--Food employee observed sanitizing washed equipment and utensils without a separate water rinse between.

--Upper interior of microwave, a food-contact surface, was observed to have food residue and was not clean to sight and touch.

--A food employee was observed touching lettuce and toast - ready-to-eat foods - with bare hands.

--Observed four tongs on oven handle where exposed to possible contamination from employee clothing and debris on oven door and handle.

--Observed full size pans of fully cooked foods such as casseroles, meat pies, etc. stored directly on the floor in walk-in cooler and walk-in freezer area, rather than six inches off of the floor as required.

--Wood pallets used to store chemicals in the dish machine area, are not smooth, non-absorbent, corrosion resistant.

--The floor/wall juncture in employee men's toilet room is not coved and closed to 1/32 inch.

--Floor under equipment in preparation and storage areas of the food facility is extremely dirty, dusty, and in need of cleaning.

--Raw shell-eggs stored over lettuce and cooked meats in the walk-in cooler.

GOLDSBORO FIRE CO. LADIES AUX.

--Food facility has not conducted a nitrate or nitrite test on non-public approved water supplies as required.

--Observed an accumulation of old-food debris on most of the soda nozzles of the customer self-serve soda machine. (Corrected)

--The final rinse gauge on the high-temperature mechanical ware-washing machine reached 157 F but never reached 180 F as required by the manufacturers data plate. Heat sensitive test strips indicated the machine is working properly.

--Observed an accumulation of dirt and dust on the floor under/behind shelving near walk-in cooler.

--Observed an accumulation of white mold-like residue on fan guards and shelving in the walk-in cooler.

--Observed an accumulation of dust, food debris, and grease on the top, sides, and under most equipment under hood system.

--Observed an accumulation of old-food debris and grease on the wall behind ovens and deep fryers in kitchen and behind ware-washing sinks in ware-washing area.

--Observed an accumulation of old-food debris on the floor under and wall near soda machines in store area.

--At time of inspection hand-washing sink in ware-washing area had low water pressure to facilitate proper hand washing. (Corrected)

--Observed several bread forms to be torn or frayed and are not smooth or easily cleanable in kitchen area.

--Food facility is using an approved non-public water system, but does not have current laboratory testing results for water potability. Water analysis can be faxed to 717-346-3229 or emailed to mneff@pa.gov

--Observed ceiling, in walk-in cooler, with an accumulation of dust.

440 N. Main St., Spring Grove

--Labels on a variety pack or picnic package of raw chicken, raw beef and raw pork over-wrapped and being sold as a single package did not include on the safe handling label to cook all the meats to an internal temperature of at least 165 F as required. Package removed from sale. Corrected on the spot.

--Soap was not available at the hand-wash sink in the meat-cutting room. Corrected on the spot.

--Possible cross contamination. Observed pinned or tenderized beef steak displayed for sale over whole cuts of beef on the retail floor. Corrected on the spot.

--Possible cross contamination. Observed ground beef and ground sausage stored over-top on whole cuts of pork in the meat cutting room. Corrected on the spot.

--Possible cross contamination. Observed individually packaged beef, chicken and pork then over-wrapped as a single package with instructions on the labeling to cook all meats to an internal temperature of 165 F. Removed from sale. Corrected on the spot.

--Observed plastic portion cups being used as scoops and stored in ham salad inside the walk-in refrigerator. Corrected on the spot.

--Observed an employee scooping ice from the bulk ice machine using a round plastic container instead of an ice scoop with a handle. Corrected on the spot.

--Observed wiping cloths soaking in a quaternary ammonia sanitizing with a concentration of zero parts per million (PPM) in the deli department. Corrected on the spot.

--Observed a roll-up receiving door that does not form a light tight seal when in the closed position.

No violations were found in the following nine restaurants during their inspections.
